Financial Responsibilities

Payment is due in full at time of service. Insurance is not accepted.

Best Self Aesthetics and Wellness, PLLC requires that you put a card on file to reserve your appointment time. Your card will not be changed at the time of booking.

Cancellations

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the nurse's day that could have been filled by another patient.

Best Self Aesthetics and Wellness, PLLC requires 48 hours notice to cancel or change an appointment.

Cancellations made with less than 48 hours notice are subject to a $50 Late Cancellation fee.

Fees

  • If appointments are canceled or changed with less than 48 hours notice, the credit card on file will be charged a $50 Late Cancellation fee. This fee is non-refundable.

  • If you do not show up for your appointment, the credit card on file will be charged a $100 No Show fee. This fee is non-refundable.

  • If you show up more than 15 minutes late to your appointment, that appointment will be canceled and the credit card on file will be charged the $50.00 Late Cancellation fee. This fee is non-refundable.

  • Please be advised that all clients/patients are subject to these policies.

Photography

Best Self Aesthetics and Wellness, PLLC will only take photographs of your treatment areas for medical purposes to be used for patient care, marketing, social media literature and /or case presentations.

Understand that:

  • Photographs are taken to capture outcomes of the procedure/treatments.

  • Photographs may be used for print, visual or electronic media including but not limited to, scientific presentations, websites, and for purposes of informing the medical profession or general public about the procedure. These uses may also include marketing on behalf of the aesthetician's practice.

  • You will not be identified by name in any of the materials or online.

  • Every effort will be made not to reveal your identity, unless you give express permission to do so.

  • You have the right to revoke this authorization at any time through a written revocation to Best Self Aesthetics and Wellness, PLLC.

  • Best Self Aesthetics and Wellness, PLLC and representatives will be released from any and all claims and demands arising out of, or in conjunction with, the use of the photography.
